AWS Membership Survey Distribution Request

On your behalf, AWS will send an email to our membership with a link to your survey. The cost is $300 for AWS members and $400 for non-members. To receive the discounted pricing, non-members are welcome to join AWS as a Regular or Associate member.

Survey requests can be submitted by an AWS full-member. Student or resident members should identify an AWS full-member as a mentor. A letter of support should be written by the mentor and uploaded during the application process.

The survey request form below must be completed with the following submissions:

  • Investigator and co-investigators names
  • A one-page (500-word maximum) abstract, including background (including evidence of prior validation of the survey tool), primary and secondary goals, methods, and anticipated results
  • Identify the survey tool, an independent web-based link through another service i.e. SurveyMonkey (AWS is not responsible for creating survey tools)
  • A link to the survey Proof of IRB approval

The AWS Member Services Committee oversees survey applications. Within a month of receipt, two AWS Member Services Committee members will review the application and communicate their decision whether to accept the request or not. Should the application be denied, the requestor can revise their application, based on the reviewers’ suggestions. The original reviewers will reevaluate the application.

After the application is approved, the requestor will be notified by AWS and payment will be arranged. A draft email, with a subject line and appropriate links and graphics, should be sent to headquarters.

AWS sends the emails on the requestor’s behalf. The requestor will never receive member emails. Surveys will be sent twice; 2-4 weeks after payment and draft email receipt, and 1-2 weeks after the original email.

If the survey results in a publication, AWS requires a copy of the final, published results and should be referenced as a source in the publication.
